Patricia R. Kozachuk, daughter of Lena and Claire Cornell of Highland Falls, NY, was born on April 24, 1944, and crossed over to the spirit world on February 14, 2020.
While living in Cornwall, NY and working at the hospital there, she met her soul mate, John Kozachuk, in the summer of 1995. Patricia moved to South Fallsburg, NY, and married long time resident, John Kozachuk, in November of 1999. They spent 25 wonderful years together.
She is survived by her husband John Kozachuk, her daughter, Holly Osborn, her son Jordan Kozachuk and daughter-in-law Katyana Kozachuk, and grandchildren, Alex, Adryana and Gabriella.
A celebration of her life will take place on, Saturday, February 22, 2020 at the Fallsburg Fire Department located at 10 Rail Road Plaza, South Fallsburg, NY. Hours 1 p.m. - 5 p.m.
